| Complete the form below, it's that easy. Once we receive your form, you will be contacted and given a date to perform. |

| Other Requirements? |
Every performer must bring 7 people to perform and admission to enter is $10 per person
For every 7 people that a performer brings, he/she receives 5 minutes of stage time up to a max of 15 minutes
18 to enter, 21 to drink
Two drink minimum with a $10 minimum per person
Performers must be scheduled ahead of time. NO WALKUPS TO PERFORM!!!
